Waiver of Liability

This Waiver of Liability (the "Waiver") is executed between Studio Guarana LLC (the "Business"), located at 4984 Thornwood Cove, Acworth, GA 30102-6955, US, and the undersigned client (the "Client").

 

WHEREAS, the Client wishes to participate in activities provided by the Business; and

 

WHEREAS, the Client acknowledges that participation in such activities may involve inherent risks, including but not limited to personal injury, property damage, or other losses.

NOW, THEREFORE, in consideration of being permitted to participate in such activities, the Client hereby agrees as follows:

 

1. Assumption of Risk: The Client acknowledges that they are aware of the risks associated with the activities and voluntarily assume all risks, both known and unknown, related to participation.

2. Indemnification: The Client agrees to indemnify, defend, and hold harmless Studio Guarana LLC, its owners, employees, agents, and representatives from any and all claims, liabilities, damages, losses, or expenses arising out of or related to the Client's participation in the activities.

 

3. Release of Claims: The Client hereby releases and discharges Studio Guarana LLC from any and all claims, demands, or causes of action that may arise from the Client's participatio in the activities, whether caused by the negligence of the Business or otherwise .

 

4. Acknowledgment of Understanding : The Client acknowledges that they have read this Waiver in its entirety, understand its contents, and voluntarily agree to its terms. The Client further acknowledges that this Waiver is binding upon their heirs, assigns, and legal representatives.

 

Image Release Form

I consent for photographs to be taken for press releases that may include name , age and city of residence. I consent for photographs and video to be taken for Studio Guarana LLC own use on social media.

 

Emergency Cancellation Policy

I understand that Studio Guarana LLC has the right to cancel and/or postpone this class at any given time at the discretion of the organization.

 

Class Deposit

I understand that there is a $50 non-refundable deposit as part of the camp registration fee.
